In addition, please download and reliably fill in the questionnaire and valuation included below in a pdf file and send it to the following address: mjakut@grupakety.com along with scans:

• License: Country, Export, Brokerage
• Agreements for the conclusion of OCS/OCPD – in full (a necessary condition. You can obfuscate sensitive data.)
• certificates of assignment/entry in NIP, REGON, KRS, CEiDG.

By sending the completed questionnaire and the above-mentioned documents, you confirm that you and the persons whose data are made available to us have read the above-mentioned GDPR information.

Please include the word “cooperation” in the subject line.

After verification of the complete set of documents, you will receive information about the next steps.
